Grace was started by someone who had cared for her own family at home, and kept seeing the same gap: people sent home from the hospital with no one to help them stay there safely. We serve families across Providence, Kent & Washington Counties with skilled nursing, personal care, and companionship.

People would be discharged from the hospital and sent home — often with no one to help them stay there safely. Grace exists to close that gap: real caregivers, in the home, for as long as a family needs them.
